Vanderbilt Assessment Scales are a tool to diagnose ADHD in children 6 and older

If your child has  first time consult for behavior, focus, or attention concerns we asked that this tool is completed prior to the visit.

Each parents should fill out a “Parent Form”.  Please also submit a “Teacher Form” to  child’s teacher(s).

Once complete, please submit these forms to our office by email them to advicenurse@potomacpediatrics.com . Upon receipt, your doctor will review and score them at which point we will contact you to schedule your behavior consult. If you haven’t heard back within 7 days of submitting your forms, please contact us for a status update.
